Delivery Method |
The user’s default delivery method of choice displays as per their default settings stored within their Relais profile. If a different delivery method is requested, select it from the list. |

Delivery Address |
The user’s default delivery address associated with the selected delivery method displays. If a different delivery address is requested, type it. |

Messaging Method |
The user’s default messaging method of choice displays as per their default settings stored within their Relais profile. If a different messaging method is requested, select it from the list. |

Messaging Address |
The user’s default messaging address associated with the selected messaging method displays. If a different messaging address is requested, type it. |

Maximum Cost |
If the user has specified a maximum cost they are willing to pay for the servicing of this request, enter it here. Note that this amount may be pre-calculated based on the default maximum cost stored within their default record. |

Need By Date |
Use the calendar to specify the required need by date. If there is no need by date requirement leave this field blank. |
|
Account |
If the user has an account to which the request must be applied, select it. The user’s default account displays if it has been set within their default record. |
|
Address Information |
The remaining fields are used to define the mailing address to be used if either the Delivery or Messaging Method is set to Mail. These include the Street address, City, State/Province, Country and Zip/Postal Code. |
