Language

Select the available language in which the Relais Access pages displays.

Department

Select the patron’s department from the list. 

Street Address

Type the patron's address.

City

City in which the patron resides.

State/Province

State or Province in which the patron resides.

Country

Country in which the patron resides.

Zip/Postal Code

Zip/Postal Code in which the patron resides.

Delivery Method

Select the patron’s preferred delivery method for documents from the list.  The delivery method may be modified on a request by request basis.

Delivery Fax

The patron’s fax number for delivering documents, automatically formatted as (###) ###-####. 

Delivery Email

The patron’s e-mail address for sending messages regarding post to web documents. 

Pickup Location

Select the default pickup location.

Messaging Method

Select the patron’s preferred method for receiving messages from the list.  The messaging method maybe modified on a request by request basis.

Messaging Fax

The patron’s fax number for sending messages.  Fax numbers are automatically formatted as (###) ###-####.  To turn off this formatting, e.g., when entering an international fax number check the box beside Messaging Fax. 

Messaging Email

The patron’s e-mail address for sending messages. 

Messaging Phone

The patron's telephone number.
